The second is the weather forecast. I fire up the computer or look at my iPhone and check the min and max temperatures for the day. I also check the local rain radar to see what's coming Finally, I look outside and check the current cloud cover. Here are my personal guidelines: 0-10 degrees: Anything under 10 degrees means that I wear booties and gloves. I also wear long sleeve "roubaix" jersey, "roubaix" bib shorts, short sleeve woollen undergarment, full length leg warmers, a wind vest and a cotton cap. If it's below 5 degrees then I'll wear ear warmers as well. I'll also swap my short sleeve woollen undergarment for a long sleeved one. If I'm going to be doing a lot of descending I'll swap my cotton cap for a thin thermal hat. 10-15 degrees: I wear a long sleeve "roubaix" jersey, normal weight bib shorts, short sleeve woollen undergarment, full length leg warmers, a wind vest and a cotton cap. 15-20 degrees: Short sleeve jersey with arm warmers, normal weight bib shorts, short sleeve woollen undergarment, 3/4 leg warmers, a wind vest and a cotton cap. 20-25 degrees: Short sleeve jersey with arm warmers, short sleeve cotton undergarment, normal bib shorts with 4/4 leg warmers, cotton cycling cap. 25-30 degrees: Short sleeve jersey, short sleeve cotton undergarment, normal bib shorts and cotton cycling cap. 30+ degrees At this point I feel like I'm almost naked! Long descending - If the temperature is lower than 25 degrees I'll always take a wind vest with me if I am going on a ride that has lots of long descending. I'll zip it down when climbing, then zip it up when descending. Successful, enjoyable bike commuting is all about being prepared. For anything. Especially weather. An investment in a good waterproof, breathable jacket and pants and neoprene booties make the ride to and from work a whole lot more comfortable (and healthier). So unless you live in the Mojave or another perennially dry place, carry your weather protection, just in case. It's Good to Be Comfortable You don't have to be decked out like a competition racer to ride your bike to work. But if the commute is longer than 3-4 miles, investing in some cycling shorts with chamois padding can make the ride a lot more pleasant. How Common Is Genital Herpes? Genital herpes is on the rise. According to the Centers for Disease Control and Prevention (CDC), since the late 1970s, the number of Americans infected with the virus has increased 30 percent to about 45 million (see Genital Herpes Statistics). The disease affects more than one in five adolescents and adults. It is more common in African Americans than in Caucasians, and is more likely to affect women than men. In the United States, one out of four women is infected with HSV-2. Compared to 20 years ago, genital herpes is about five times more common in Caucasians ages 12 to 19 and twice as common in adults ages 20 to 29. Symptoms of Genital Herpes Most people have no or few symptoms of genital herpes. When symptoms do occur, they typically appear as one or more blisters on or around the genitals or rectum. The blisters then break, leaving tender ulcers (sores) that may take two to four weeks to heal the first time they occur. Typically, another outbreak can appear weeks or months after the first, but it is almost always less severe and shorter than the first outbreak. Although the infection can stay in the body indefinitely, the number of herpes outbreaks tends to decrease over a period of years. The Herpes Sore Within a few days of early symptoms, sores (also called lesions) show up where the virus has entered the body, such as on the mouth, penis, anus, or vagina. Sores can also show up on a woman's cervix or in the urinary passage in men and women. The sores are small red bumps that may turn into blisters or painful open sores. Over a period of days, the sores become crusted and then heal without scarring. Sometimes, there is a crack or raw area or some redness without pain, itching, or tingling. The total time from the beginning of symptoms to sores appearing is usually around two to four weeks. What Is Subclinical Shedding? Genital herpes can reactivate without causing noticeable bumps, blisters, sores, or other symptoms. Sometimes, symptoms are there but are so mild that they aren't noticed. This is called subclinical shedding. During these times, herpes can be transmitted to an uninfected sex partner, even though there are no symptoms. Subclinical shedding is most frequent in the first year after the initial infection, but it continues on and off for several years. Most new cases of genital herpes are caught from a person with subclinical shedding. This is because people with genital sores often avoid sex, but those with subclinical shedding don't know it. As for recurrent outbreaks, subclinical shedding is more common due to HSV-2 than HSV-1. Recurrence of Symptoms During the first outbreak of herpes symptoms, the virus travels to nerves that are near the spine. The virus remains inside the nerve cells in a latent or dormant state. While the virus is dormant, a person has no symptoms. Periodically, the herpes virus becomes active once again, leading either to a recurrent herpes outbreak or to subclinical shedding of the virus. About 90 percent of people with genital herpes caused by HSV-2 will have a recurrence during the first year (the average number of outbreaks is four). About 55 percent of people infected with HSV-1 will have a recurrence of genital herpes during the first year (the average number of outbreaks is less than one). Over time, these recurrences usually decrease in frequency. Recurrences are generally much milder than the first outbreak of genital herpes and typically last one to two weeks. Symptoms with recurrent infections might include: Itching Tingling Vaginal discharge A burning feeling or pain in the genital or anal area. What Is Genital Herpes? Genital herpes is a sexually transmitted disease (STD) that normally affects the genitals, buttocks, thighs, or anal opening. It is caused by an infection with the herpes simplex virus (HSV). HSV is in the same family of viruses that cause HSV type 1 most commonly infects the mouth and lips, causing sores known as fever blisters or cold sores. It is also an important cause of sores to the genitals through oral sex. Up to 30 percent of genital herpes cases are caused by HSV-1. HSV type 2 is the usual cause of genital herpes, but it can also infect the mouth through oral sex. Is There a Cure? Preventing a herpes infection in the first place is always the best medicine, because there is no cure for genital herpes (see Preventing Genital Herpes). Once you have the virus, it stays in your body, and there is a chance that you will have herpes outbreaks. Medication can shorten and stop outbreaks from happening, but it cannot cure the condition. HIV and Genital Herpes There is growing evidence that people with genital herpes are at greater risk of getting HIV (the virus that causes AIDS) if they have unprotected sex with someone infected with HIV. People with HIV who are also infected with genital herpes may have more frequent and severe outbreaks, and their episodes may be more difficult to treat. An Introduction to the Signs and Symptoms of Genital Herpes When a person is infected with genital herpes, symptoms do not begin right away. During this incubation period, a person will feel perfectly fine. However, about two weeks after being infected, signs and symptoms may begin. When symptoms do occur, it is known as a herpes outbreak. Not everyone infected with herpes will have an initial outbreak. In fact, it is common for people to have no symptoms with the initial infection -- but they can still spread herpes! Early Symptoms A person who does have early signs and symptoms of genital herpes may have a variety of symptoms. Some of these early symptoms can include: An itching or burning feeling in the genital or anal area Flu-like symptoms, including fever, headache, a general ill feeling (malaise), and muscle aches Swollen glands Pain in the legs, buttocks, or genital area Vaginal, anal, or urethral discharge A feeling of pressure in the area below the stomach Pain or itching during urination Constipation or difficulty urinating. For some people, these symptoms can be mild. For others, initial symptoms can be quite severe. In order to make a diagnosis of genital herpes, a healthcare provider begins by asking a number of questions and performing a physical exam. The condition can often be diagnosed by looking at the sores. However, some cases are more difficult to diagnose, especially in between outbreaks. In these cases, other tests may be recommended, including a viral culture of a lesion (if one is present) or a blood test. Treating Genital Herpes While there is no cure for genital herpes, several prescription medications are effective in speeding up healing and in helping to prevent future outbreaks. They can also decrease the risk of passing herpes to sexual partners. These medicines are known as antivirals. Antiviral medicines approved to treat genital herpes include: Acyclovir (ZoviraxÂ®) Famciclovir (FamvirÂ®) Valacyclovir (ValtrexÂ®). Fever, headache, muscle aches, painful or difficult urination, vaginal discharge, and swollen glands in the groin area are less common with a herpes recurrence than with the initial outbreak. Some people experience red bumps that become blisters and then open sores, as with the first episode of genital herpes. However, in recurrent outbreaks, there are usually fewer lesions and they don't last as long as the first outbreak; pain usually lasts for three to five days, and healing usually occurs in 10 to 14 days. Recurrent lesions can occur anywhere on or near the genitals, but the most common sites are: The penis in men Near the vaginal opening or on the labia in women Near the anus or on the buttocks of either men or women. Recurrent outbreaks usually appear at the same location every time. Some people report that stress, other illness, or menstruation bring on an outbreak of genital herpes. However, research indicates that these factors, in fact, have little effect; recurrent outbreaks probably occur randomly. In any case, recurrent episodes usually are not predictable. Sometimes, the virus can become active but not cause any visible sores or symptoms. During these times, small amounts of the virus may be shed at or near places of the first infection in fluids from the mouth, penis, vagina, or from barely noticeable sores. This is called asymptomatic (without symptoms) shedding. Even though you are not aware of the shedding, you can infect a sexual partner during this time. Asymptomatic shedding is an important factor in the spread of herpes. Genital herpes is caused by an infection with the herpes simplex virus. This sexually transmitted disease affects more than one in five adolescents and adults. During a herpes outbreak, tender ulcers will generally appear around the genitals or rectum. While there is no cure for genital herpes, many medications are available to help prevent and shorten outbreaks. What Should I Do If I Have It? If you think you have genital herpes, you should do the following: See your healthcare provider for testing and treatment right away. Follow your healthcare provider's orders, and finish all the medicine that you are given. Even if the symptoms go away, you still need to finish all of the medicine. Avoid any sexual activity while you are being treated for genital herpes and while you have any symptoms of an outbreak. Be sure to tell your sexual partners so that they can be tested and treated, if necessary. Remember that genital herpes is a lifelong disease. Even though you may have long periods with no symptoms, you can still pass the virus to another person. Talk with your healthcare provider about what you can do to have fewer future outbreaks and how to prevent passing the virus to another person.
